It reads the HTML, because that is what they read

ChatGPT's and Claude's crawlers do not run JavaScript. Text a browser adds after the page loads is not there when they fetch it, so the scan scores the version they see and says when the two differ.

An empty alt is not a missing one

alt="" is the correct markup for a decorative image, and counting it as a fault tells you to write captions for spacer graphics. Only images with no alt attribute at all are counted against you.
